Transportation Services
We can arrange for transportation to and from appointments that are approved by the Ohio Medicaid PASSPORT Waiver, assist you with making transport plans, and connect you with our transportation coordinator for further help.

Other Services
Though able to live at home on their own, our clients often need assistance with activities associated with daily living such as brushing their teeth, showering safely, or running errands. Our care teams are trained to provide individualized services for each client based on their needs.
We offer:
